To book an appointment, please Contact Us by phone or email. We value one-on-one evaluation before accepting appointments, as we want to ensure we approach each patient and treatment on an individual basis.
You may pay online for services or gift certificates. You can now pay for your appointment by selecting a product below, but will need to book by phone or email.
When booking your appointment, please:
- Complete the Health Intake Form and bring a hard copy with you if you are able.
Special Circumstances
- 1-hour emergency session (off-hours) may sometimes be arranged. Fee is variable.
- In-home and in-hospital appointments may sometimes be arranged.
Notes for Booking & Billing:
- Treatment fee can be applied to your Health Savings Account (HSA) or Flex Spending Account (FSA) just ask for a receipt.
- A $20 fee is levied for any returned checks.
- At least 24 hours’ notice required for changing or cancelling an appointment.
- Fee for missed appointments or cancellations less than 24 hours in advance (except emergencies) is $40.
